ABSTRACT

ABSTRACT Insects are fundamental to biodiversity conservation in almost all ecosystems, and their population decline, and extinction directly result from environmental impacts. These facts are aggravated by the lack of knowledge of insect biodiversity, the so-called biodiversity deficits, especially the Linnean and Wallacean shortfalls. In freshwater ecosystems, biodiversity loss is higher among aquatic insects, and caddisflies comprise one of the most vulnerable orders. In this way, research focusing on describing new caddisfly species and understanding their distribution ranges will increase knowledge of caddisfly biodiversity. In the past 14 years, the team from the Laboratório de Entomologia Aquática "Prof. Dr. Claudio Gilberto Froehlich" (LEAq, Universidade Federal da Bahia, Brazil) and collaborators have been addressing these issues, resulting in 55 described species of caddisflies. Taxa in other insect orders have also received attention and an additional 16 species have been described (eight mayflies, four stoneflies, four neuropteran spongillaflies, and antlions). Here, eight caddisfly species are described and illustrated (Atopsyche froehlichi sp. nov., Austrotinodes zeferina sp. nov., Cernotina kariri sp. nov., Neoathripsodes froehlichi sp. nov., Notalina claudiofroehlichi sp. nov., Oecetis marcus sp. nov., Phylloicus froehlichi sp. nov., and Polycentropus claudioi sp. nov.), five of them in honor of LEAq's patron. In addition, new distributional data are presented for 10 known species. A checklist of the caddisfly fauna of Bahia state is also presented, with 138 species, around 75% and 30% of them recorded and described by the LEAq team, respectively. Currently, for caddisflies, Bahia is the fifth most species-rich state in Brazil, and the first in the Brazilian Northeast region.

ABSTRACT

Pediatric patients have particularities in the clinical manifestations and complications of snakebite envenomation (SBEs), and few studies have examined this population. The objective of this paper was to study snakebites in a pediatric age group treated at a reference hospital and to evaluate factors associated with unfavorable evolution. A cross-sectional study with a clinical-epidemiological description and identification of the factors related to unfavorable evolution in patients aged <19 years old seen from January, 2018 to November, 2019 was performed. Complications related to the SBE, such as compartment syndrome, secondary infection, extensive necrosis, hemorrhage, and kidney damage, were considered unfavorable evolution. From the 325 patients in the sample, 58 were aged <19 years old; 40 (69%), 0-12 years old; and 18 (31%), 13-18 years old. All patients had local manifestations (mild, moderate, and severe), and 36 (62%) had an unfavorable evolution. Fourteen (24.1%) patients had compartment syndrome, with a significant risk association between 0 and 12 years old (p = 0.019). Two factors significantly contributed to unfavorable evolution: the timing from the bite to medical care being ≥6 hours and additional antivenom therapy needed. We conclude that the younger the patient, the smaller body segment affected, leading to disproportionality between the affected area and the amount of inoculated venom, contribute to more frequent local manifestations and complications in children rather than adults. Because of the relationship between body area and vascular volume in children differs from that in adults, the same volume of venom inoculated by snakes will be disproportionate in these two groups. Therefore, in the treatment of pediatric patients, increasing the volume of antivenom therapy is possibly necessary. Furthermore, as in adults, six hours between the bite to medical care increases the risk of complications and mortality.

ABSTRACT

ABSTRACT This review aims to provide current information about Q fever, elucidating the etiological, epidemiological, pathogenic, clinical, diagnostic, therapeutic, and prophylactic aspects of the disease for the medical community. We discuss the main forms of presentation of the agent, its ability to persist in the body, the infinite possibilities of susceptible hosts, the main known forms of transmission, its importance in populations at occupational risk, and the role of arthropods in the natural history of the disease. Focusing on Brazil, we present the cases already described and studies developed since its first report, and how there is still much to unravel. We are aware of the possibilities of the persistence of the agent and the development of severe clinical pictures and the specific treatments currently instituted. We also wish to raise awareness about the future, the new genotypes that are emerging, the need to study the effects of vaccines, and the impact of Q fever on the population. Q fever is a poorly understood disease in Latin America, and recent studies, especially in Brazil, have revealed the importance of developing new studies.

ABSTRACT

Abstract The pathogenesis of systemic lupus erythematosus (SLE) is complex. Few studies in Brazilian population have addressed cell phenotypes associated with immunological responses and their associations with SLE activity. The aim of this study is to investigate cell phenotypes associated to SLE diagnosis, treatment and activity. Twenty-eight SLE female patients (17 inactive, 11 active) and 10 healthy women were included in this study. Markers of natural killer (Nk), T and B cells in peripheral blood were evaluated by flow cytometry. Nkt cells were decreased only in SLE active patients. Activated CD4+, regulatory T FoxP3+ and B cells were decreased in both active and inactive SLE patients, compared to control group. The data corroborate the disruption of immune regulatory response in SLE patients and suggest phenotipic changes as possible biomarkers of SLE activity.

ABSTRACT

Abstract Objectives: Upper respiratory tract infections in children generally have significant morbidity and mortality. There is little data available about functional immaturity of the immune system and the child's susceptibility to infections at the beginning of their lives, thus, justifying a more specific immunological analysis. Method: Analysis of hemograms and innate and adaptive immune responses in 95 children between age 1 to 6 years with episodes of recurrent respiratory infections (test group n = 39) and without these episodes (control group n = 56) was carried out. The production of reactive oxygen intermediates by peripheral blood cells stimulated by phorbol myristate acetate was analyzed. Additionally, the number of B lymphocytes, auxiliary T lymphocytes, and cytotoxic cells was determined using flow cytometry. Results: Results from both groups did not show statistically significant differences in red blood cells, total leukocytes count, and the differential neutrophils, eosinophils, basophils, lymphocytes, and monocytes count. The analysis of the number of B lymphocytes, auxiliary T lymphocytes (LTCD4), and cytotoxic cells (LTCD8) also did not show any difference between both groups. However, the production of radical oxygen intermediates was significantly reduced in the test group as compared to the control group (p < 0.05). Conclusions: There was no difference in the analysis of hemograms, leukograms, or the number of lymphocytes, LTCD4, LTCD8, or LTCD19. The reduced production of oxygen intermediates in the affected group suggests that these children's microbicide capacity is compromised, which may be related to their recurrent respiratory infections.

ABSTRACT

A feared fungal disease surprised and became a warning to severe cases of COVID-19, especially to health professionals involved with the pandemic. Designated as black fungus for public health services in India, where reported data reflects an increase of more than eighty times the expected increase for Rhizopus among the communities. The disease has become even more worrisome due to the high mortality already established as an opportunistic infection, coupled with the reserved prognosis for all those infected and hospitalised by the SARS-CoV-2 severity criteria. This patient, who was submitted to corticosteroid therapy, in an excessive dose, therefore immunosuppressive, developed a severe, disseminated clinical form. It was verified the progression of the lesions and thus the high risk of trans- surgical lethality, or, also, by the insufficiency of conduct in removing the lesions to their satisfaction. Thus, the therapeutic option is the associated use of micafungin, liposomal amphotericin B and isavuconazole for the regressive phase. The patient remains hospitalised with progressive and discrete improvement. Until the opportunity of reevaluation of the surgery by the interspecialty collaboration.

ABSTRACT

Introduction: Atopic dermatitis (AD) is characterized by chronic itching, presenting with fluctuating evolution, resulting in sleep disorders and social stigmatization due to the presence of visible and recurrent lesions, that might become progressively lichenified. Factors such as those mentioned and others associated, such as incapacity for work, lack of concentration throughout the day and isolation have profound impacts on the patient's mental health, resulting in low self-esteem, depression and frustration. In addition, it is known that AD is essentially an inflammatory disease. And, recent studies demonstrate the role of inflammatory cytokines in the development of depressive syndromes, therefore may be a causal correlation between the conditions by inflammatory pathways. Objectives: This systematic literature review aimed to analyze a relationship between atopic dermatitis and depressive symptoms, identifying mechanisms responsible for this connection. Methods: The research was carried out between 11/17/2020 and 11/18/2020 following the PRISMA model and using PUBMED, Virtual Health Library (VHL) - IBECS, LILACS and CUMED - and EMBASE databases. Keywords "Depression" and "Atopic Eczema", along with its MeSh and DECS terms, were used and associated using the Boolean method. Inclusion criteria were defined as articles that are clinical or observational trials involving a group of patients with atopic dermatitis and a control group, which could be constituted by the group with atopic dermatitis itself, however, after an intervention. The depressive symptoms had to be measured by scales or, at least, the criteria for diagnosis of depressed syndrome must have been established. Results:In the end, fifteen studies remained, which were classified among those that compare treatments and their outcomes related to depression and AD, those that propose AD as an aggravator of depressive symptoms, those that propose depressive symptoms as an aggravating factor for AD and those that bring statistical analysis without clearly establishing where the causality relation resides. Conclusion: Several studies have presented a relationship between the condition of AD and depressive symptoms in distinct pathways, or analyzing AD as an aggravating factor for depressive symptoms or vice-versa. From this perspective, there may be a cyclical bidirectional causality, in which constant positive feedback generates worsening of both conditions until the adequate approach is taken, highlighting the importance of the multidisciplinary propaedeutics for these patients (AU)

ABSTRACT

Triatomines are blood-feeding arthropods belonging to the subfamily Triatominae (Hemiptera; Reduviidae), capable of producing immunomodulatory and water-soluble molecules in their hemolymph, such as antimicrobial peptides (AMPs). In this work, we evaluated the antifungal and immunomodulatory activity of the hemolymph of Meccus pallidipennis (MPH) and Rhodnius prolixus (RPH) against Cryptococcus neoformans. Methods: We assessed the activity of the hemolymph of both insects on fungal growth by a minimum inhibitory concentration (MIC) assay. Further, RAW 264.7 macrophages were cultivated with hemolymph and challenged with C. neoformans. Then, their phagocytic and killing activities were assessed. The cytokines MCP-1, IFN-γ, TNF-α, IL-10, IL-12, and IL-6 were measured in culture supernatants 4- and 48-hours post-infection. Results: Both hemolymph samples directly affected the growth rate of the fungus in a dose-dependent manner. Either MPH or RPH was capable of inhibiting fungal growth by at least 70%, using the lowest dilution (1:20). Treatment of RAW 264.7 macrophages with hemolymph of both insects was capable of increasing the production of MCP-I and TNF-α. In addition, when these cells were stimulated with hemolymph in the presence of C. neoformans, a 2- and a 4-fold increase in phagocytic rate was observed with MPH and RPH, respectively, when compared to untreated cells. For the macrophage killing activity, MPH decreased in approximately 30% the number of viable yeasts inside the cells compared to untreated control; however, treatment with RPH could not reduce the total number of viable yeasts. MPH was also capable of increasing MHC-II expression on macrophages. Regarding the cytokine production, MCP-I and TNF-α, were increased in the supernatant of macrophages treated with both hemolymphs, 4 and 48 hours after stimulation. Conclusion: These results suggested that hemolymph of triatomines may represent a source of molecules capable of presenting antifungal and immunomodulatory activity in macrophages during fungal infection.(AU)

ABSTRACT

ABSTRACT Clinical similarities among viral diseases become even more relevant considering the current scenario, especially in Brazil, where there is a high incidence of these diseases and overlapping seasonality. We report the case of a patient with acute clinical manifestations composed of predominant respiratory symptoms and alveolar hemorrhage in which three etiologies (dengue, influenza and COVID-19) were investigated concomitantly. Only the diagnosis of dengue was confirmed. Then, the patient's immunological profile in response to stimulation of mononuclear cells with dengue virus antigen was analyzed in an attempt to identify specific characteristics that could be associated with the clinical manifestation.

ABSTRACT

ABSTRACT Background: Domestic pigeons carry pathogens in their droppings, posing a potential public health problem. Methods: The phenotypic and genotypic antimicrobial resistances of Staphylococcus aureus and Enterococcus faecium in the feces of urban pigeons near hospitals with intensive care units were measured. Results: Twenty-nine samples showed Enterococcus growth, whereas one was positive for S. aureus. The S. aureus isolate was sensitive to the antibiotics tested via antibiogram, however resistance genes were identified. E. faecium isolates showed phenotypic resistance to gentamicin, erythromycin, and ciprofloxacin. Conclusions: Antimicrobial profiles harmful to health were demonstrated in bacterial pathogens isolated from the external environment of hospitals.

ABSTRACT

House call is a powerful care management tool that brings the Family Health Unit closer to the territory in a lively and dynamic manner. Institutional support contributes with the teams to qualify and operationalize this important instrument that expands user-centered care. This paper is a qualitative, descriptive, experience repot study about the role and perception of the institutional support offered to a health region in the municipality of Camaçari, Bahia, Brazil, during a house call to a family belonging to the enrolled territory of a Family Health Unity in January 2020. The presence of institutional support in the team's work process contributed to direct the path chosen for the health care of a socially vulnerable family, helping to resume care, enable the family-team interaction, and strengthen the intersectoriality between health and social development. The report reveals that some challenges, especially the reinvention during the new coronavirus pandemic, the importance of implementing other Health Care Network posts, expansion of the debate on health promotion, and the co-responsibility in care production, must be faced with the teams, so that it results in enhancing the quality of life of users belonging to the home and territory.

ABSTRACT

ABSTRACT The presence of abnormalities in knee alignment (genu varum and genu valgum) is extremely common in soccer. It can occur in the junior league / youth academy and perpetuate itself throughout the professional career. The consequences of years of high-level sports practice in players with genu varum / genu valgum can be harmful to the knees. By observing football matches of the present and the past, the authors of the article noted that great Brazilian soccer players were, or are, affected by the malalignment of the knees: Garrincha (the most famous), Leandro, Dunga, Romário, Rivaldo and Neymar. This study aims to discuss the relationship between high-performance sport and the development of these orthopedic deformities, serving as a tool for updating the trauma-orthopedic knee subspecialty. Level of Evidence III, Retrospective comparative study.

ABSTRACT

Mycobacterium leprae and Mycobacterium lepromatosis are gram-positive bacterial pathogens and the causative agents of leprosy in humans across the world. The elimination of leprosy cannot be achieved by multidrug therapy alone, and highlights the need for new tools and drugs to prevent the emergence of new resistant strains. Methods In this study, our contribution includes the prediction of vaccine targets and new putative drugs against leprosy, using reverse vaccinology and subtractive genomics. Six strains of Mycobacterium leprae and Mycobacterium lepromatosis (4 and 2 strains, respectively) were used for comparison taking Mycobacterium leprae strain TN as the reference genome. Briefly, we used a combined reverse vaccinology and subtractive genomics approach. Results As a result, we identified 12 common putative antigenic proteins as vaccine targets and three common drug targets against Mycobacterium leprae and Mycobacterium lepromatosis. Furthermore, the docking analysis using 28 natural compounds with three drug targets was done. Conclusions The bis-naphthoquinone compound Diospyrin (CID 308140) obtained from indigenous plant Diospyros spp. showed the most favored binding affinity against predicted drug targets, which can be a candidate therapeutic target in the future against leprosy.(AU)

ABSTRACT

This was a cross-sectional, anonymous, online survey aimed at assessing the perceptions and basic knowledge of COVID-19, a highly transmissible disease caused by SARS-CoV-2, in a sample population in the Triângulo Mineiro region, Minas Gerais, Brazil. A questionnaire devised by the researchers and distributed through social media was applied between June 16, 2020 and August 21, 2020. The survey consisted of questions about the basic aspects of COVID-19, which included symptoms, risk groups, suspicion of infection, prevention, transmission, and perception regarding social isolation. The average distribution, frequencies, similarities and differences between the responses for the different variables were evaluated. Five hundred twenty valid responses were obtained from participants aged ≥18 years. Most of the respondents showed satisfactory basic knowledge of COVID-19. Moreover, the data showed that the participants scored an average of 87.6%. Sex, age, and socioeconomic vulnerability presented a statistically significant link with knowledge of the disease; women, young participants, and the least socioeconomically vulnerable had the highest scores. This study indicated that the population in the Triângulo Mineiro region able to access social networking platforms were basically well informed regarding COVID-19, although differences were observed depending on the group analyzed.

ABSTRACT

ABSTRACT Nuclear power plants are designed with systems dedicated to offering a high degree of protection to their workers, the population, and the environment. Between the levels of protection of the population and the environment, there is the nuclear emergency plan. This plan must contain a specific segment related to the management of risk communication to the population. Historically, risk communication techniques have emerged to minimize harm to people, through population panic control strategies. However, this issue is currently underdeveloped in Brazil in the field of nuclear safety. Thus, this work aimed to establish an initial proposal on the use of the theory of Incident Command System as a strategy for managing risk communication in the event of accidents at the Almirante Álvaro Alberto Nuclear Power Plant located in Brazil.

ABSTRACT

Introduction: Interferon-gamma (IFN-g) signaling is mediated by crosstalk of receptors, such as IFN-g receptor 1 (IFN-g R1), transcription factors, such as signal transducer and activator of transcription 1 (STAT1) and suppressors of cytokine signaling 1 (SOCS1). Here, we evaluated the role of IFN-g signaling in peripheral blood mononuclear cells (PBMCs) from asthma patients and control individuals. Methods: PBMCs from adult healthy nonasthmatic controls (n = 12; male and female, 18-60 years old) and patients diagnosed with asthma (n = 18; male and female, 18-60 years old) were stimulated with IFN-g (0.25, 0.5 and/or 1.0 ng/mL) and, after 24h, the production of CXC motif chemokine 10 (CXCL10) was evaluated (by enzyme linked immunosorbent assay) as well as the expression of IFN-g R1, STAT1 (both by flow cytometry assay) and SOCS1 (by real-time qPCR assay). Results: CXCL10 production was reduced in a dose-dependent manner in PBMCs from asthma patients stimulated with IFN-g when compared to control individuals. While IFN-g induced an increase in IFN-g R1 expression and phosphorylated STAT1 (pSTAT1) activation in PBMCs from the control group, a reduction in both IFN-g R1 and pSTAT1 was observed in PBMCs from asthma patients. IFN-g increased SOCS1 mRNA expression in PBMCs from asthma patients when compared to IFN-g-stimulated cells from control individuals. Conclusion: Taken together, our results demonstrated that IFN-g signaling is downregulated in asthma patients.
